• A completed/signed Q-Active New Dealer Application **
• A Uniform Sales & Tax Use Certificate filled out and signed or your state form if not listed on the federal form **
• A copy of your Business Certificate of Liability Insurance **
• A copy of your business phone bill or yellow pages ad **
• Photos of your commercial location including storefront, sales area, and service area **
